Business Office Forms
Please submit your completed forms to the appropriate email address below, or put it in an envelope with your name/department and ID/account number and leave it in our Business Office drop box to the left of the Student Accounts Office in Humanities 110. Call 717-867-6300 or ext. 6300 for assistance or to schedule an appointment.

Lebanon Valley College is required to provide you with a 1098-T form if your student was enrolled at least part-time for academic credit in order for you to be eligible to claim either the American Opportunity or the Lifetime Learning Credit on your federal tax return.
Additional information on educational tax credits and Form 1098-T may be available through your tax accountant or by referring to the Internal Revenue Service’s web site.
1098-T forms will be mailed each January by the last business day of the month.
Lebanon Valley College is offering you the opportunity to retrieve your 2017 Form 1098-T online. We have contracted with a specialized provider for this service.
IRS regulations state that each student who wishes to receive their 1098-T electronically must also provide his/her consent electronically. Therefore, if you wish to take advantage of this you must visit www.tsc1098t.com. The instructions are outlined below:
- Go to www.tsc1098t.com
- Enter the Site ID: 11442
- User Name: Student ID
- Password: Last 4 digits of your Federal TIN (Tax ID Number)
For security reasons, you will then be prompted to change your password. Enter your old password (last four digits of your TIN) and then enter a new password. The new password must be 7 characters and contain at least one numeric character.
Once you have entered your new password, you will be presented with the online consent form. Read the Online Consent Form for the requirements and specifications. If you agree, click on “Consent”. Your consent will apply to 2018 and subsequent years.
We ask that you take advantage of this new service to reduce costs here at Lebanon Valley College and lessen the environmental impact.
